These Terms apply to website usage only. They do not govern the doctor–patient relationship or clinical services, which are covered under separate agreements and HIPAA regulations.

Clinical services are provided only through secure platforms and are governed by separate Telehealth Informed Consent, HIPAA Notice of Privacy Practices, and other clinical policies—not these website Terms.
Accessing telehealth platforms requires your agreement to their terms and privacy practices.

Appointments may be scheduled:

Submitting an appointment request does not guarantee availability or confirm a physician–patient relationship until formally accepted by the Practice.
Website forms should not contain sensitive medical information.

Payments for services may be made via:

We do not accept insurance plans at this time.
